While BODYWISE is credentialed with most major insurance companies, check your health plan to be certain massage therapy is covered.

Not all employers opt for massage in their benefit package - please call the number on the back of your card to verify coverage.

Fees are payable in full at the time of service, including insurance clients.

Currently, First Choice, Group Health, and Regence are not accepting applications from massage therapists.

Prescription Required

A prescription for massage is now required by nearly all insurance companies. The following information is necessary for benefits to be paid:

Frequency

Number of visits

Start date

End date
